I am able to get the form to upload to my website. however when i press submit at the end it doesn't take me to the 'Thank you' page and i don't receive an email for it. Do you know whats wrong with it?

Could you try submitting the form using the form's direct link? If it works, then the problem is how you have embedded your form.
Could you please provide us more information on how you have embedded your form to your website? You can share us the link to it. We advise our users to embed the form using the iFrame embed code in case the default one does not work. Just found out that it's working fine on Internet Explorer and it's not submitting on Google Chrome.

I test submitted your publish form http://www.jotform.co/form/61191194576865 on your Wordpress website and I was able reproduce the problem as well.
The form was stuck to "Please wait.. " and did not submit.
it seems that our publish code for Worpress have issue. I checked the browser console and I see red error log regarding the problem.
To fix the problem, please try re-embed process using instead the iframe embed code of the form.
Follow this steps:
#1 Remove the embedded form in your website. Make sure to clear any script of the form on the page.
#2 Publish again the form using its iframe embed code . Follow this user guide to get the iframe code.

#3 Save your wordpress page after adding back the form.
Then test the form again on the website.
