Shared form disappeared

I recently updated a form that was shared with me called Kalahari 2016 and it appears to have disappeared... I am in need of updating the form and I cannot locate it! 

Is there anything that can be done to help me? The individual who shared it with me, is not at work today and I really need to make a few adjustments... this is the link to the form. https://form.jotform.com/Gleaner1894/kalahari2016

I see that that form is under the account username Gleander1894. I checked his account and the form is not anymore shared to your account. Unfortunately, we cannot make any changes on our users account for security and privacy concerns. We can only do that if the request is directly made by the user himself/herself. It would be best to contact the said user and ask to share the missing form to your account again. The form is actually in his/her account's Archive folder.
