    Our form shows as disabled when we try to view it

    I haven't placed this form on a site, rather using it as a comp example for someone. However, when I share the URL it shows as disabled. Can you help? Here's the link to the form:

    I took a look at your account and I see that it is disabled due to the following form: "Hotel Booking Form"

    It seems as login form, and this is not allowed per our terms of use.

    As such we can enable your account, but only after you or us remove the same form.

    Thanks so much for your response. I grabbed that form from the templates and was just tinkering with it. I've deleted it, could you please re-enable my account?

    Thank you for your update. Unfortunately, when reviewing your account. I see that the form in question still exists. Please delete the form so we may re-activate your account.
