On the form I have created, it shows two boxes for phone number.

When I preview the form or go to change it there is only one phone number box. How can I correct this?

Phone Number field, by default, has an area code field together with the phone number field, that is why you're seeing two textboxes for the phone number field. However, that can be changed by enabling the "Input Mask" option of the field.
