Image is too big for the form

Hi there, I have followed the advice above but my form still looks like this, can you help?

Please adjust the width and the height of the image. Just select the image field and then click the Width button in the toolbar, put 620 and then press the OK button. After that, click the Height button and put 400. 

Width: 620
Height: 400
