Account suspended - not notified

    Our account has been suspended and we don't have access to this important form

    but we weren't notified of suspension and don't why, please advise.

    I've checked your forms. It seems your account was suspended due to asking credit card information.

    Please note that asking credit card information is not possible with JotForm and asking credit card information which is against our terms of use. You can setup a form to collect details such as name, address, email address and other non-sensitive information. I've deleted those fields and re-activated your account.

    If you want to create payment form you need to create our Payment tools to ask payments. Please check the guide below for more information:
