You can easily create a form like that.
Just click on form builder tool called Upload File from the left bar , then go to Show Properties and you can customize the size limit of your form.
Once on the general settings, click on the field next to Max File Size and type the desired amount in Kb.
Keep in mind that 1024 KB = 1MB , so for example if you want to allow a maximum size of 25 MB to be uploaded, you need to enter 25600 in that field .
Hope this work for you!

Thanks Jeanette. :)
The max file upload size is 1 gig. The limitations may depend on what account type you have:
'Free' account type users have 100MB of upload space.
'Premium' users have 10GB of upload space.
'Professional' users have 1TB of upload space.
File uploads can be of any type and the form builder can define the file types allowed.
Hope that helps, let us know if you need any clarification on that.
