Why my forms are disabled?

Biz Funding form has been disabled. What's the reason why?

I think this is because you have some fields in your forms that may not be allowed in JotForm, on these forms http://www.jotform.us/form/61066186277158, http://www.jotform.us/form/61057194655157  you have this field: 

Do note that it is not allowed, you cannot ask for any login credentials, this is why your forms have been disabled, I have removed that field in order to enable your forms and your account.

I also found that you are asking for Social Security Number in your forms something that may not be allowed, but depends on how you are using your forms, here is a thread where you will find info about collecting SSN and why this may and may not be illegal and considered as phishing: http://www.jotform.com/answers/500652. 

I have enabled your account and your forms and you should be able to load them now, also take a look to our terms in order to know what info you should not ask for in your forms.
