When filling out a patient history form, the patient noted that the lock at the left side of the address bar appeared unlocked. The patient has a concern about security for her data. Please explain how this can be addressed as I understood page is secure.

If you are referring to one of your form https://www.jotform.us/form/60248797414160 that's because of this background image https://www.jotform.com/images/styles/style1_bg.gif from the theme you're using that is not served over HTTPS. I have now fixed this for you by injecting the following CSS codes into your form which uses HTTPS on the image URL.
Your form is now showing the security indicator icon in the address bar.
I have also updated the background image on the theme you're using so you can use it again without any worries. :)
