    Account is suspended and forms are disabled

    Second month in a row, my forms have all been disabled. I use the forms for business purposes and it's really inconvenient if I need to through this monthly. Can they please be re-enabled and can it be explained to me how to avoid this happening?

    Sorry for the inconvenience. I noticed again that your account was suspended by our phishing application. While reviewing a couple of your forms I noticed that you are asking users for their USERNAME. Unfortunately, this violates our terms and is the reason our application is suspending your account.

    Please adjust your forms so it will not ask for any type of credentials so this won't occur again.

    All cases of the word USERNAME has been changed.

    Your form 60939116086864, named Clone of Pirates vs Vikings: Points Tally, seems to still be asking for usernames:

    Please make sure that none of your forms are asking about any details such as usernames, and we will be happy to reactivate the account.
