I am trying create a form that has unique submissions and Jot form can block duplicates, which is great but I want to make sure the submissions entered are the same as distributed entry codes for a contest. This will keep someone from just typing in any random code, and making sure it matches the codes I have distributed through the school.

The condition should work exactly as you have it. I believe you can have 10 per condition. Here is my test form with a similar setup to your example:

It is only a single condition for all 3 values.

Okay great, I have enter 23 codes so far on one condition, and it seems to be accepting them.

Awesome, I am not sure the limit but it is nice to know 20+ will work. If there is anything else we can help with, just let us know.
