Need template for developing a form for art association.

They offer multiple classes.

I want to be able to have photo, with headlines, description, and cost depending on membership.

I want to be able to students to be able to select multiple classes and the form total and them be able to pay.

Can the form also send them a receipt with a supply list for class they are taking?

NO URL, as I am trying to get them to go digital.
