When I "send a test email" from my form I receive a test, from JotForm, but when the form is actually submitted I am not receiving an email.  I have included a screen shot with my email screen settings.  Please advise.

Your actual form submissions are sent based on our logs. You can check this in your account history. Here's how: https://www.jotform.com/help/293-How-to-view-all-your-form-Email-History. Since the emails are sent, they may have been filtered as spam to your end so I'd suggest checking that.

If nothing, try to uncheck the Attach PDF from the advanced settings and make an actual submission again. If still nothing, try to switch back your sender email to noreply@jotform.com keeping the attach PDF unchecked.

If you wish to keep the custom sender email, I would suggest adding that by SMTP. Here's how: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form. You need to get the SMTP credentials from your hosting provider.
