    Can you please try to re-embed the form using the this script code:

    <script type="text/javascript" src="https://form.jotformeu.com/jsform/61253925998369"></script>

    If it is still the same, please try this iframe code:

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotformeu.com/61253925998369" frameborder="0" style="width:100%; height:539px; border:none;" scrolling="no"> </iframe> <script type="text/javascript"> window.handleIFrameMessage = function(e) { var args = e.data.split(":"); var iframe = document.getElementById("JotFormIFrame"); if (!iframe) return; switch (args[0]) { case "scrollIntoView": iframe.scrollIntoView(); break; case "setHeight": iframe.style.height = args[1] + "px"; break; case "collapseErrorPage": if (iframe.clientHeight > window.innerHeight) { iframe.style.height = window.innerHeight + "px"; } break; case "reloadPage": window.location.reload(); break; } var isJotForm = (e.origin.indexOf("jotform") > -1) ? true : false; if(isJotForm && "contentWindow" in iframe && "postMessage" in iframe.contentWindow) { var urls = {"docurl":encodeURIComponent(document.URL),"referrer":encodeURIComponent(document.referrer)}; iframe.contentWindow.postMessage(JSON.stringify({"type":"urls","value":urls}), "*"); } }; if (window.addEventListener) { window.addEventListener("message", handleIFrameMessage, false); } else if (window.attachEvent) { window.attachEvent("onmessage", handleIFrameMessage); } if(window.location.href && window.location.href.indexOf("?") > -1) { var ifr = document.getElementById("JotFormIFrame"); var get = window.location.href.substr(window.location.href.indexOf("?") + 1); if(ifr && get.length > 0) { var src = ifr.src; src = src.indexOf("?") > -1 ? src + "&" + get : src + "?" + get; ifr.src = src; } } </script>

    If that didn't work, please remove the script element below the iframe code.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotformeu.com/61253925998369" frameborder="0" style="width:100%; height:539px; border:none;" scrolling="no"> </iframe>

    Let us know if using those embed codes didn't work. Thank you.

    I checked your webpage (http://www.mypractice.org.uk/) source code and I didn't see any JotForm codes on it. Is it possible that you weren't able to save the changes?

    Where is the form exactly embedded? Is it in the Homepage, Contact page or Warning Card page?

    Also, please use the iframe code without the script code. I checked this page ( http://www.mypractice.org.uk/amber-warning-card/) source code and I saw that it only has the script code, no Iframe code.

    Please use this code:

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotformeu.com/61253925998369" frameborder="0" style="width:100%; height:539px; border:none;" scrolling="no"> </iframe>

    The form is embedded on the 'Amber Warning Card' page.

    I've added the iframe code and updated the the page, but still nothing.

    I checked the site: http://www.mypractice.org.uk/amber-warning-card/ and I noticed that the form was loaded but was not displayed due to some CSS setup. iFrame was hidden and opacity was set to 0.

    Please try to use the iFrame code below and see if it will fix the issue. I have added some CSS code to the iFrame code to overwrite the current CSS/style that is implemented on the iFrame.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotformeu.com/61253925998369" frameborder="0" style="width:100%;height:539px;border:none;opacity: 1;visibility: visible;" scrolling="no"> </iframe>

    You just have to update the form in the Form Builder and save it. The changes you made in the Form Builder will reflect on your website. The reason is that every time you refresh your webpage, it will pull up the form from JotForm.

    Hope that helps. Thank you.
