Our clients are unable to access form

There is a problem with our TCPR5 Audition Signup Form - Error message states: THIS FORM IS UNAVAILABLE

 

This form has been working great until now.  It is critical that we get it fixed as we have a deadline for signups.

 

Please advise how to proceed.  Thank you.

The problem is, the form as been disabled due to the expiration date.

 

You can remove/extend the expiration or enable the form from MyForms page.
