    How to set check box that opens up multiple check boxes if selected yes?

    I would like to have in the form a question that says "Would you like add Middle School Sports?" with a yes or no box and if they click no nothing happens, but if they click yes then a check list of sports would show that they could select the sports they want.

    Can someone help me accomplish that in my form? Thx

    I added two more radio buttons with 'Yes' and 'No' and I added conditions that will show or hide sport fields based on selection:
