Hi I need to include a field for identity card (or passport) and i can´t. I tried it with a text box but when I fill the field, come empty.

When reviewing the form I noticed that you have a field call RUT. I am assuming this an identification number given to everyone in your country.
I notice that you make this field required. This will obligate all users to fill in the field.

On regards of the empty field, I just tested it and it worked: https://www.jotform.com/help/269-How-to-view-Submissions
It was not possible to submit it empty since the field is required. Please try again.

Also, you could embed the form using the Iframe method to avoid any problem that could cause the bypassed of the required feature.
Get a fresh Iframe code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:

The fromulario works. I was not choosing all the fields (I missed the rut)
in the Notification options and so I do not get the form with data rut.
