I have tried to find an account, but I had no luck, please provide us with your username or with the link to one of your forms so we will be able to check the form and activate your account, also if you are able to log in remove all those fields related to CC information and use one of our payment integrations instead: http://apps.jotform.com/category/payment_processing.
Also, take a look to our terms you will find that there is stated what you cannot ask in your forms and that your account will be suspended if you are asking for this kind of information: http://www.jotform.com/terms/.
Provide us with the link to your form or your username, we will check your account.
