Did you guys delete anything from our forms?

What are you referring to delete something from your forms? 

I think this is because some of your forms were disabled due to phishing, and you were assisted about this on this thread: https://www.jotform.com/answers/831806-Why-my-forms-are-disabled-. 

This is most probably the reason why you see that a field has been deleted from your forms, besides that we have not allowed to change your forms, only on cases where your forms are suspended due to phishing as the one above.
