I get an error of "one or more fields are required" when I SUBMIT the PDF fillable form, even though all required fields are filled. This happens in Chrome and in IE.
Also, in Edge, I can't enter anything in any of the fields.

I was able to reproduce the problem when I test the PDF form version of your form http://www.jotform.us/form/61148649819165
I found the caused to be the *Required Captcha field on the form which is not present on the PDF Form version.
Widgets, captcha and some special fields are not included in the PDF Form version. So you will have to exclude the captcha field on the form and create the PDF form again for it to work.
Let us know if this did not resolve the issue.

I decided to use only the Web Form and forego the fillable PDF since it would require too much maintenance of different versions of the form.
