I want to create a survey form to get family information.
The way I want it to work is to have a field that asks how many adults in the family. Then, it should present the fields (tel number, email id, etc.) and make some of them required according to the number entered. Same for children field.
I know how to do this using conditions. However, I don't know how to make the fields required ONLY when they satisfy the condition. Is that possible?
Also, I believe there might already be a template for what I need since this seems standard. Could somebody point me to the correct template for this. I tried to look but there are so many, it is a little overwhelming.
Thanks for any help.

You can search templates by categories, so, you can click on the survey category: https://www.jotform.com/help/106-How-to-Use-the-Form-Templates
If you want to start a form from the scratch, then I would recommend you using the Configurable List widget: https://widgets.jotform.com/widget/configurable_list
In this way, the user can determine the amount of members to register, example: https://form.jotform.co/61284548158867
You can clone my example form and use it if you want to: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL

Thanks, that works. I should have thought of this simple option.
Now, I have two other questions regarding this:(1) I want to set only one of the adults entries as Primary Contact. I can add a 'Primary' column dropdown. However, is it possible to create some conditions so that there is no more than one primary?(2) How do I create an autoresponder email that sends a response to the primary contact after submission?
