How to find a survey form in the templates galery.

I want to create a survey form to get family information.

The way I want it to work is to have a field that asks how many adults in the family. Then, it should present the fields (tel number, email id, etc.) and make some of them required according to the number entered. Same for children field. 

I know how to do this using conditions. However, I don't know how to make the fields required ONLY when they satisfy the condition. Is that possible?

Also, I believe there might already be a template for what I need since this seems standard. Could somebody point me to the correct template for this. I tried to look but there are so many, it is a little overwhelming.
