I would like to allow someone to hear a song but only after they agree to NDA

You can create Conditon rules on your form that will allow the end-user to be able to have access to the song only after they answered/agreed to an NDA in the form.
Let me create a demo form also that shows this. I'll get back to you shortly.

What did you use to embed the song?Now how do i get a copy of the NDA and the name that agreed to hear the song. For instance they could just exit out of the form after hearing the song. Does it have to be 2 separate forms or can they hit submit and stay in the same form then the option to play song is presented. Also is there a way to track how many times the song is played or is there a widget allowing that kind of tracking.

I did check my colleague's form and found that he added a condition to show hide the text field with player. Please check the screenshot below:
