Now how do I get a copy of the NDA and the name that agreed to hear the song?

    Now how do i get a copy of the NDA and the name that agreed to hear the song. For instance they could just exit out of the form after hearing the song. Does it have to be 2 separate forms or can they hit submit and stay in the same form then the option to play song is presented. Also is there a way to track how many times the song is played or is there a widget allowing that kind of tracking.

    I have checked the form provided on the original thread, I can see that the form only has the text and the embedded song, there is no submit button, it means that will not be possible to get a copy of the text and the other user's info. 

    First, I noticed that you are using a Text field to show the text in your terms, this field cannot be included in the email notification, you will need to use a widget, for example, these widgets may help you to display the terms and include the text in your email:

    Short_scrollable_terms: This widget will allow you to include the text in the email notification, but I have tested it and it is not possible to use conditions with this widget, I mean, it has its own check box, but this cannot be used with conditions. 

    Narrative_fields: This will allow you to display the text and will be able to include it in your email notification or auto-responder, but the text on this widget cannot be formatted as you can do with the text field that you're currently using in you form. 

    If you don't like these widgets, you will need to copy and paste the text in your email template, otherwise it will not be included. 

    Regarding to capture the submitter's data, I can see there is no a submit button, it means that the form cannot be submitted and emails cannot be triggered, you will need to add a submit button in order to send emails, now, I'm not sure if you want to add the text boxes that will capture the user's data in the form where the audio is displayed, if you don't want to display the fields there and you want to redirect your users to another form, then this guide will help you with that:

    You will also need to add a submit button to the form, once the audio finished your user will have to click on the submit button and then will be redirected to the other form where the data will be captured.
