JotForm is a free online form builder which helps you create online forms without writing a single line of code. No sign-up required.
We believe that if one user has a question, there could be more users who may have the same question. This is why many of our support forum threads are public and available to be searched and viewed. If you’d like help immediately, feel free to search for a similar question, or submit your question or concern.
Unfortunately, it is not possible.
If you want to add text areas that ask input from your users, you can do that by using the default "Text area" fields found under the "Basic Fields":
The text area field allows you to set a hint example, and you can also switch from plain text editor to a rich text editor, that will allow you users to format there text:
Unfortunately, you cannot add them on a table, although it is possible to format them in one line, you can check this guide on how to position your fields in your form: https://www.jotform.com/help/328-How-to-Position-Fields-in-JotForm
You can also try using the matrix field, it gives you a table like format and allows you to ask inputs from your users, however, it doesn't have a textarea field.
Another advance field that you can try is the Configurable list widget, it allows your users to add multiple entries with different inputs, it's in a table like format. You can format it and set a minimum number of rows to make it look like a fixed table: https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget
Let us know which one works for you best. We will then help you on formatting it.
