May I know if you have checked your submissions page "Incomplete Payments" list? If you received the payment in Paypal but it was marked as "Incomplete payment" in your submissions page in JotForm, then it is highly possible that your Paypal's IPN is not enabled.

I also checked your form and I see that the "Paypal Account" text box has a combination code in there? I am not quite familiar with this, usually the merchant Paypal email is the one added here? Please do make sure that the Paypal email address of your merchant account is the one entered here.
Let us know if the above suggestions help resolved the problem, basically you need to enable your Paypal's IPN and make sure that the merchant Paypal email address is entered in the "Paypal Account" text box. We'll wait for your response.
