Unable to publish my form

I used your test form to create the survey form that i needed.  Now it will not allow me to publish.  I did not have any of these problems when I began using Jotform in March.  Please help.

I'm not sure if I understand your question correctly. Are you experiencing any issue with publishing your form? Do you see any error message? As I check your JotForm it seems to be loading fine at my end. We request you to go through the guide below that can help you in publishing the form.
