Thank you very much for your reply. If possible I would like to insert the
following optin form into an openoffice document then convert that document
    to a PDF: https://form.myjotform.com/61296525476565
Your time and assistance are greatly appreciated.

If I understand you correctly, you would like to insert the form into your open office document and print it as a PDF.

I did a small research and I do not thin it is possible to insert web forms into open office, You can try creating a PDF form and then try inserting it into your open office, but I also do not think this is possible.

    Hi Victor,
Thanks much for your replies and your time and consideration. I am trying
to learn how to create flipbooks that I can sell and when I do that I will
    upgrade to a paid plan.
Based on your last email it sounds like I should give up on putting my form
into an openoffice document, so can you tell me if it will be possible to
creat a PDF then put the optin form in it as well as video embeds and
hyperlinked images? Or if possible would it be best to just use Jotform to
create a PDF from scratch and include those elements in it?
    Best Regards, Ron

Thanks for your reply Beryl. Sorry if I am being redundant but is it
possible to create a PDF from scratch and embed a video in the PDF?
