Add a sinup form to aPDF document

Hello sorry to have to ask but I checked the FAQ and the User Guide and I could not find out how to add a signup form to a PDF document. Can you advise?

If I understand you correctly, you would like to insert the form into your open office document and print it as a PDF.

I did a small research and I do not thin it is possible to insert web forms into open office, You can try creating a PDF form and then try inserting it into your open office, but I also do not think this is possible.

Thanks much for your replies and your time and consideration. I am trying
to learn how to create flipbooks that I can sell and when I do that I will
upgrade to a paid plan.
Based on your last email it sounds like I should give up on putting my form
into an openoffice document, so can you tell me if it will be possible to
creat a PDF then put the optin form in it as well as video embeds and
hyperlinked images? Or if possible would it be best to just use Jotform to
create a PDF from scratch and include those elements in it?

If you're trying to upload a video on you fillable PDF form I am sorry to say that it is not possible with Jotform. Please, check the guide below for more information about fillable PDF form.

Unfortunately, that is not supported in PDF forms.
