How an I change the form name of a theme form I used and modified?

Need to change the Summer Camp registration forms and remove  3-5th graders.  Only shows in the test registration email name....

To change the form's title you will need to click on the Preferences icon, you will find it on the top toolbar while on the Form Builder, then you will get a window, there you will be able to find two options where you can change the form's title, the first one is the name that you set to the form, this will be shown in My Forms page, the second is the form's title, the one shown in the web browser's tab, it is the same as the <title> HTML tag. 

These are the only options to change the form's name, if this does not help, please provide us a bit more about what are you looking for and we will be glad to assist you.

Thank you it didn't work for either option... I'm trying to change the
theme form name from "Summer Camp Registration 3rd-5th Graders"... Which is
the title of the email notification of the submitted forms.

I understand that you want to change the subject line of the email notification. You may change it from the Email setup wizard by clicking on Emails icon > Edit Notification. 

Please see the screencast below that can help you in changing the subject line for your notification.
