We apologize for the inconvenience. We had a replication issue on our servers in EU, which may be why the form isn't displaying properly on the preview. Rest assured our developers are working on it.
However, I did check the form you have linked and the forms appears properly on my end. Can you try saving the form again and checking it in the Preview to see if the issue still persists?
