I have tested your form and email address that I entered inside of your form was sent successfully to your email address. You can also see it inside of Inbox folder on your account.
Inside of this guide you can find how to view your email history or logs:
https://www.jotform.com/help/293-How-to-View-All-Your-Form-Email-History
Considering that your email field is not required, your users probably submitted your form without entering any data. You can prevent them to submit blank form by making your field required. To do that inside of your Form builder select your email field and click on Required option.
Hope this will help. Let us know if you need further assistance.

That's a log of your Email History for you to view and track the details of emails sent out from your account to and from you via your forms from users after they hit our servers.

That is Autoresponder email that is set inside of your form http://www.jotform.us/form/61055511548149.
Autoresponder email is automatically sent to your users once your form is submitted. If you do not want that Autoreponder is sent to your users once they submit your form, you can delete Autoresponder email from your form. Inside of this guide you can find how to do it:

Does someone have access to your account? If so maybe they have added Autoresponder email to your form. I have checked Form Revision History of your form and I saw that your form was updated today.

Did you cloned that form to your account? If original form had same Autoreponder email, your cloned form will also have it.
