Creating a registration form with installment plans

    Question about form flexibility

    Hello, I am interested in a registration form for use for our religious education classes this fall, which would enable us to get people to fill in the parent info, then student info (Multiple students on one form is great (with their grade level next to it...i.e.:

    Student Name                     Grade level (Pull down)

    Student #2 Name                Grade level (pull down)

    Then, it would have the ability to pay (using some a company that offers credit cards or e-checks)

    Then, the ability to say "bill me" with choices of total bill or bill me in 2 installments, 3 installments or 10 installments.

    I am attaching something similar to what I want.  I guess my question is does your company offer the opportunity to use a template (or build it from scratch) to offer this kind of registration?  Thanks and I look forward to hearing back.

    I am afraid that form needs to be created from scratch. The layout will also be different, but you will be able to control it.

    I would like to suggest checking our form templates to get the idea about web forms created at JotForm.

    We do support payment integrations on forms. A recurring payment option may be helpful if you would like to provide installment plans.
