I Can't Find My Form!/2 Payment Options

Hello,

I created a form awhile back and now I want to edit it but I cannot access it.  However, when I preview a theme, it gives me the option to view with my forms and the form appears there.  How can I view the form so that it can be edited? I have included a link of a screenshot that shows where I can see the form.

Also, is it possible to have 2 payment options (Paypal & Stripe) on the same form?  I want to give customers a choice.  If so, how do you do it?
