Why are my forms disabled?

Why are my forms disabled? Please let me know.. 

Your forms were disabled because your account was autosuspended.

This is because you have a form that is asking for log in credentials. Below is a screenshot of it. We do not allow users to collect these kind of information, JotForm forms are designed to work as a log in system. Forms like this is highly suspicious as a phishing page which violates our terms of use.

Please check our terms page here: http://www.jotform.com/terms/. Please do read the section "Phishing" and "Collecting Sensitive Information". We have an anti-Phishing system and a dedicated team of manual reviewers to check our users forms, violations will lead to account suspension and possible for a permanent IP ban.

For now, I have deleted that form and re-activated your account, please edit your forms and avoid creating similar forms that violates our terms if you wish to continue to using our form builder. Please also make sure that deleted forms that violated our terms are "Purged" in the "Trash" folder. Thank you.
