Kindly take a look on this link that may help you and guide you on how to used the page break.
If incase it doesn't work, try to re-embed your form into your website using Iframe. Here's the tutorial on how to get you form's Iframe: http://www.jotform.com/help/148-How-to-get-your-Form-s-Iframe-Code

It also maybe because you have a condition that prevents it from going to the next page.
- kffdnAnswered on March 12, 2012 at 03:12 PMHi,
Thank you for your response. Yes, I have looked at that link before, but it doesn’t tell you how to troubleshoot the problem. The page will still not advance.
Also, the break is in place, and information below (a page collapse) is showing on an earlier page than it should. Ideas?
Finally, I removed all conditions for the entire form in case this was affecting the “next” button.
Thanks for your attention to this matter,

Hello Katie. After you have removed the conditions, was your form able to work on your end?

According to my tests, there is some problem with the Primary E-Mail Address field title on your form. I have moved some part of title to the Hover text, and now the Next button is working fine on my test form.
Please give it a try and let us know if you need any further assistance.

Please also note that Form Collapse elements may conflict with Page Break elements, and we can not guarantee that these elements will work together in the same form.
