I recently received an email saying "You have currently reached the limits of your account. Your forms are disabled now."
I don't think that I reached the limit of 100 submissions, so I tried to login and see my account details, however it said "No user with this email address or username has been found."
I signed up with jotform.net, but I received the following link to upgrade http://www.jotform.com/upgrade?username=wakako (jotform.com) Do you think that has something to do with this issue?
My form disappeared from my website.
Please let me know what the problem is.

I checked your account and found out that it has reached allocated limit. Kindly note that you have 100 submissions, 10 SSL submission and 10 Paments whichever gets full first for your current account.
So in your case, you have used 14 SSL submissions and this is overlimit. Your forms have been disabled till you upgrade to to higher account (PREMIUM) to enjoy more allocation or till the end of month when the account will be reset.
