Try deleting the email and re-creating it. Here is how to do it:
Open the form on the Form Builder and
1. Click on "Setup and Share" tab,
2. Click on "Email Alerts",
3. Select "Notification" and click on "Delete Email",
4. Click on "Email Alerts" again,
5. Click on "Add New Email" and select "Notification".
Deleting the email and re-creating it fixed the problem. THANKS.
