Forms are not working

Links to my forms are saying they are disabled but they aren't. How do I fix this?

Your account has been automatically suspended and your forms disabled because your form is used to collect sensitive information( Credentials - email, password), which is not allowed - JotForm Terms of Use. 

For security reason, you cannot create form that collect certain type of information. To re-enable everything back, the form must be deleted, or you can change the purpose of the form.
