Why is my form disabled?

Everytime i change the background on my form, its suddenly disabled.  Can you please help me fix this issue?

This is because you are asking for usernames in one of your forms, this is the form ID where you have these fields #61275175783160.

I had to remove those fields in order to activate your account, but please take a look to our terms, there is stated that you cannot ask for usernames, passwords or CC info, this is why a form can be disabled and an account suspended: http://www.jotform.com/terms/. 

Please avoid to use words like username or password as well, even if you're not asking for this info in a field, but you have this text in your form it will be disabled, this will help you to avoid your forms be disabled again.
