While editing my forms, they suddenly shut down all forms on my website. Last year I encountered a similar problem and when I contacted Jotform through their facebook page the forms were quickly re-instated and was told "an over-zealous tech had mis-understood the use of the site". I've been using these forms for years and don't use them for phishing or anything remotely unethical. I don't use the jotform credit card forms because of the many specific requests contained on my forms and because this option didn't exist when I first set up the forms. Can you assist me with this? We do not allow collecting sensitive credit card information:
"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
You need to delete those fields from your form, otherwise, you account shall remain suspended.

I assure you I am not "deceptively" obtaining credit card information. I am PCI compliant, use the secure form format, and have transacted this way for many years. In April of 2015, this same thing occurred when "Kirin" suspended my account, but after communicating through jotform through facebook, my forms were re-instated. There is no deception here. Please re-instate my forms. Thanks, Paul

The account/username sloanp is Active and available. So you can do change on your forms.
Login at https://www.jotform.com/login and do the suggested modifications on the form to avoid getting them suspended by the Phishing filter.
