If you would like to remove the jotform branding from the form's url and use your own business name instead, you can do so by using Custom Domains. Simply follow this guide to set it up: How to use Custom Domains with Jotform.

You would have to contact your hosting service, Dotster, and ask them to setup up a DNS so it will automatically redirect to the form when entering the URL.

musicandhearingReplied on May 17, 2016 at 5:44 PMSo I thought I was done with this project but I am using the cloned form by someone there- I renamed it my biz name patient registrar - when it populates an address for it there is an eu I address and form isn't loading into zapier???? Ugh
