How can I have a customized URL that says my biz name instead of JotForm?

    Also am wondering if the form can have a customized URL that says my biz name instead of jotform?

    If you would like to remove the jotform branding from the form's url and use your own business name instead, you can do so by using Custom Domains. Simply follow this guide to set it up: How to use Custom Domains with Jotform.

    I'm confused- do I do this at Dotster that hosts my domain or through your site?? I'm not linking it to my website. Thanks

    You would have to contact your hosting service, Dotster, and ask them to setup up a DNS so it will automatically redirect to the form when entering the URL.

    So I thought I was done with this project but I am using the cloned form by someone there- I renamed it my biz name patient registrar - when it populates an address for it there is an eu I address and form isn't loading into zapier???? Ugh
