My form is not working

This is because you have inserted an invalid CSS code in your "Custom CSS" tab in the Terms & Conditions widget. Here's a screenshot:

That widget let's you add a LINK to your terms and conditions page. The Custom CSS is used to style the widget. To learn more about CSS, here's a link to it: http://www.w3schools.com/css/css_intro.asp 

If you want to manually type or insert the text in the widget, then what you might need to use is the Short Scrollable Terms widget.

Or you can simply paste the terms in a "Text" field.
