May we know if the form is embedded on your website? Or can you share us the exact link that your customers are viewing?
I tested your form using the direct link which is this one https://form.jotformz.com/61368834048664. And viewed it on an iPhone 6S device, it works as expected and is mobile responsive. Here's a screencast of it:
I presume the form is embedded on a website? If it is, please do make sure that you have enabled your form's mobile responsive options, here's a guide on how to do that: https://www.jotform.com/help/322-How-to-Make-Mobile-Friendly-Forms-on-JotForm
Now, if it is indeed embedded, then please try re-embedding using our JotForm iFrame embed code. Here's how to get that embed code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Let us know if that above solutions worked.
