Issues with JotForm and Adobe Sign

Hello,

I have set up the Adobe Sign widget on my Jotform. Everything works well and populates well except the HTML structure that is converted into the signable document. I have spoken to Adobe and they have informed me that the issue is on the JotForm side not Adobe, as the document is created by Jotform. 

Please let me know how I can fix this issue asap, so I can start using the form!
