Why is my forms disabled?

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I notice that your JotForm "Debit Order Instruction" is collecting Credit/Debit card information using plain text fields which is not allowed with JotForm. Such forms are considered as phishing forms and associated accounts are suspended automatically. Please refer to our Terms of usage for more information on Phishing forms. 

    I have now removed the guilty fields from your JotForm and activated your account. Please refrain collecting sensitive information as mentioned in our terms to avoid any further suspensions on your account.
