I'm unable to add a reply-to email to the notification or autoresponder emails

I'm unable to add a reply-to email to the notification or autoresponder emails. It's not showing either a name or email field when I try to setup a notification. the form is JM Free E-Book Sign Up Form.

I checked the form and 'Reply-to-Email' is working properly now.

There was a temporary issue recently with email notification but it has been fixed, this is probably the cause.
