When checking into this app fine out why it creates blank submissions. I had 31 and never used it.

The creation of 31 submission within the imported form happened because the spreadsheet that was imported had data filled up to its row 32 (row 1 contained the headings, so row 2 was the first submission, and row 32 was the 31st submission):
If you want to import a form without any submissions, please make sure that the spreadsheet you are importing contains nothing on any other row, except for the headings in the very first row of the spreadsheet.
For example, when importing a sheet that has two rows, the first one will be considered a heading (for creation of fields), and the second row will be imported as 1 submission:
And upon importing it to your forms, the app will clearly state how many submissions it imported from the sheet:

Here is a form created with Excel very similar to the one you looked at. So it appears that only one line forms can work not these.

One other thing of note, the "Data" you saw was not actually data but field descriptors. All the information is actually one "form" with no data.

You are correct that one line forms can work for the spread2jot app. Seeing your image, I can see that the app may encounter issues when reading the rest of the data of your file since some rows also have multiple rows within them. This could cause the form to experience blank submissions or have 31 submissions that are never used as you have encountered. For the app to work properly, the headings that will count as the fields will need to be in a single row, since any row after that will be treated as submissions.
