I am trying to integrate a Jotform with Mailchimp. I have followed all the instructions listed on your site, but when testing out the campaign on mailchimp, I do not see the jotform attached.
If you can please advise as to how I can make sure that the jotform appears when sending out the mailchimp email.

Based on what you have mentioned Mollie, I believe that you were following the steps mentioned here: How to Integrate with MailChimp
Now, in regards to your tests. Did you use the same email?
Please note that you will not be able to add the same email multiple times, unless you delete it, but even then, you could have issues if you try to use the email that is associated with your MailChimp account.
What I would suggest is to test your form and MailChimp integration with a new email.

Yes I was following the instructions that you noted.
In the tests yes I used the same email, but I am still not sure how to get the jotform to appear on my mailchimp mailer.
Is there any way to contact someone over the phone?

Now, I see you've removed the integration from your form, I would suggest you to integrate your form with MailChimp again and make some tests, once you've received a submission this data should be added to your list in MailChimp as a new contact, if this is not the case, do let us know and we will be glad to help you.
