I used a template and when I go to publish on FB it has the other persons info

Can you please provide us more information regarding this concern, such as the link to the fb page, the form used, etc?  I have checked your form but I did not find something unusual with it.

When I go to publish and click on the facebook app the link that shows on my profile says Oklahoma Poodles, this is what I am trying to change. I cannot find the tab to change that title. Thank you 

While editing your form, click on the preferences tab to bring up your form preferences menu.  From there, click on the "Advanced" tab, then click on the "Show More" button at the very bottom.

That will show the "Page Title" field.  Update that to your preferred title and click "Save Changes" to update the title.
