If I have a paper form I use for injury & treatment data, can I import it without using form builder? What are the options please? Thank you.

There is no way to import a hard copy form to web form. But if you have an excel spreadsheet of the data you have, you can try importing the data to JotForm using Spread2Jot. Although in my opinion, I'd rather create the form via the form builder. It would be better to do your layout with the preferred fields you have in mind.
