Why some of my fields in submissions disappeared?

Also, on some forms that I get back some questions do show up at all while on some completed forms where the same questions have the same or no answers (no check boxes are checked) the questions still show up. I can't understand why some of my questions are disappearing.

Can you help me correct these two problems?

I checked few of your submissions, I couldn't find any missing questions. You have few fields with checkboxes that are not required so maybe your customers didn't populate those fields?

If you can please give us some example submissions so we can review them it would be helpful in our investigation.
